  • You'll work with our medical provider and a health coach to dive deeper into your health history to identify the root cause of your symptoms. Once they know the root cause, your medical team can diagnose your underlying condition and treat you. Together, you’ll develop a health plan for long-term care focused on healing.

    GETTING STARTED

    15-minute discovery with a physician.

    Complete in-depth medical questionnaires that provide insight into your overall health.

    Meet with your medical provider and health coach. The care manager will be there to answer any questions about your membership.

    Have a 75-minute initial appointment with your medical provider to understand your health concerns and receive initial recommendations.

    Have a 90-minute initial appointment with your health coach, who will work closely with your doctor to recommend healthier habits tailored to your medical needs and lifestyle. Your health coach will help support you in implementing these changes, offering support every step of the way.

    ASSESSMENT & INSIGHTS

    Complete provider-recommended labs and advanced specialty testing tailored to your health goals.

    Review results with your medical team and receive recommendations for how to treat your symptoms and heal.

    Get your personalized health plan to address any underlying conditions with appropriate medical treatment, including recommendations for better nutrition, sleep, stress management, healthy movement, and incorporating supplements and medication as needed.

    TREATMENT & PROGRESS

    Begin your personalized health plan, including nutrition, stress reduction, movement, sleep optimization, supplements, and medication, if necessary.

    Get additional annual appointments with your medical team (5 medical visits and 9 health coach visits) plus the initial visits. Stay in touch with your team through your Practice Better Portol.

    Reassess your plan and conduct additional testing as needed.

  • $550 for an initial visit with the physician and health coach & $199 per month for membership.

    An annual membership is $2,739 upfront. Or you can pay in monthly installments of $199 over 11 months ($2,739 /annually).

  • Yes, as a Sage Health member, you have unlimited messaging with providers.

  • Your Sage Health membership is set to auto-renew every year. One month before your membership is up for renewal, we’ll send you an email to discuss your renewal options. If you have any questions about the different options or want to chat about your membership, please don’t hesitate to get in touch.

  • Yes, you can add on as many additional visits as you'd like to your membership. Each additional visit is $350 for a clinician and $110 for a health coach.

  • There are some costs that are not included in your membership and may require out-of-pocket expenses:

    Functional Lab tests: Many tests are not covered by insurance. Specialty tests (like stool, urine, and saliva) are not included in your membership.

    Health care services outside of what is specified above, such as vaccinations, urgent care, MRIs, etc.

    Non-medical services recommended by Sage, such as acupuncture, therapy, fitness classes, and supplements.

  • Sage Health is available through telemedicine throughout the entire United States.

  • No, Sage Health does not accept insurance.

  • Many of our members use their FSA/HSA to pay for clinical fees, specialty tests, and supplements. However, we recommend checking with your FSA/HSA company/carrier for more information and details as to how you can use these funds. In case there is any additional documentation you need, we’re more than happy to help get you the necessary paperwork.

  • At Sage Health, our membership is set up as an annual plan with the option to pay in monthly installments or in a single upfront payment. All members have the opportunity to cancel their membership after 6 months.

    If you no longer wish to be a member after 6 months, we will cancel your membership. Once your membership is canceled, you will no longer have access to the providers or services.

    We do not offer refunds on our annual or monthly membership plans before the 6 month period.
